1. Pharmacy Disclosure & Medication Notice
Medications provided through Renovate4Wellness, including compounded semaglutide, are dispensed by:

Important Notice: Compounded semaglutide is not approved by the U.S. Food and Drug Administration (FDA). It is prepared by a licensed compounding pharmacy based on documented medical necessity and prescribed off-label by a licensed provider. Patients must understand and consent to this prior to receiving treatment.
2. Telehealth Consent Summary
By receiving services from Renovate4Wellness, you acknowledge and agree that all medical consultations and follow-ups are conducted via telemedicine, which involves electronic communication with licensed healthcare professionals.
Benefits of Telemedicine:

Flexibility in scheduling
Secure consultations from your chosen location
Limitations of Telemedicine:
Inability to perform a hands-on physical exam
Potential technical or connectivity issues
Limited emergency care capabilities
A separate, signed telehealth consent form is required before any services or prescriptions are provided. Telehealth is not appropriate for all conditions. In case of a medical emergency, dial 911 immediately.

4. Pricing, Payment & Refund Policy
Renovate4Wellness operates under a self-pay model. We do not accept insurance.
All fees for services and medications are clearly disclosed prior to purchase.
Medication charges are non-refundable once dispensed.
Any unused, prepaid services (excluding medications) may be eligible for a partial refund upon written request, subject to review.
By purchasing any product or service, you acknowledge and agree to these payment terms.

6. Eligibility & State Coverage
Renovate4Wellness currently provides services only to residents of Pennsylvania, where our licensed healthcare providers are authorized to practice.
Patients must be physically located in Pennsylvania at the time of their consultation. Services will be expanded only as provider licensure is updated and verified in additional states.
